Seems like it's some sort of digital intake form for a healthcare organization. The answer related to speaking with a doctor had me curious and googling "Epic healthcare" showed that company with matching logo as a healthcare software company.

May be helpful information to share with a doctor but understandable to say no for anyone who wants to confirm their doctor is LGBT+ friendly/trustworthy first before disclosing details.

none, but they do have different odds, and more importantly they change the advice the practitioner gives them.

it would be stupid to advice the usage of a condom during anal penetration to a lesbian couple for example.
