He didn't kill the Joker. It's a pretty important plot point that despite psyching himself up to finally do it, he's still unable and the Joker somehow finishes off his neck break and taunts him that even though he couldn't do it, everyone will think he did.

IMO there are just two questions to ask. Is she in pain? Is she ever going to get better? If the answer to the first is yes, and the second is no, then it's time.
