Heyyyy my fellow Mass Effect player!
I'm on my 24th playthrough of the first game. I don't always go through the whole series because the first one is my favorite game of all time but I play it at LEAST once a year.
I periodically return to TES III: Morrowind or Gothic 1 & 2.
Deus Ex, for damn sure.

There's only one Deus Ex.
Any Fallout game but mostly New Vegas, I kind of gave up gaming when the 360 reached end of life then when the Switch released I had to have one. Zelda and the Witcher 3 and Skyrim were great on it but the lack of decent games left it languishing in a drawer.
I bought a Steam Deck last year and it completely blows the switch out of the water. I absolutely love it. I don't have the time to get set up in front of a TV and play a console but the deck is top class. Pick it up and put it down. Considering I mostly play 5 years or older RPGs it is the ultimate machine for me. I've even been playing EA golf games via emulation that run great. I even got Dopewars running on it.
But yeah New Vegas all the way, I'm currently playing Fallout 4 mind u.
Skyrim but I feel like that's a free pass for most people. Besides that, Bioshock always seems to re-appear on my desktop and get a playthrough.
Ocarina of Time. Sometimes emulated, some times OG N64 and sometimes on the 3DS. Fav game of all time
I replay Knights of the Old Republic 1&2 every so often. They're just so cozy to get wrapped up in.
I'm not sure multiplayer games count here, I always go back to this party game called "Ultimate Chicken Horse". It's quite simplistic, but I got TONS of hours out of it so far.
It's a rotation: Deus Ex, System Shock, E.Y.E, System Shock 2.
Hoping to add Peripeteia to the list this year.
Wazhack. Still haven't beaten it. It's a traditional roguelike (cf. nethack) presented in the unusual format of a 3D sidescroller. The dev is prolific about updating it year over year. Trust me on this one, game is a mix of hilarious and infuriating
Huh. Thanks -- I enjoyed it, but wasn't aware that it was still being updated. May have to take another look and see what's new.

Still the best couch multiplayer shooter
I've been playing Civilization IV & Nevewrwinter Nights fairly steadily since they came out.
Old School RuneScape! An addiction is hard to break..
I replay all the fromsoft rpgs on their corresponding anniversaries.
City of Heroes and Ultima Online (Outlands).
Night in the woods. It resonates with me a lot, plus Greg rulz
Uncharted series is fun to go through. Civ 5 also, but I have to be careful I don’t lose three days of my life.
For me, it's Path of Exile and I play it about 4 or 5 times a year whenever a new season rolls around. The novelty wears off after about a week or so but it's always nice to see what the new seasons hold
Earthbound and Super Metroid. I love the games so much. I also do The Legend of Zelda: A Link to the Past every other year or so.
Been playing it less and less lately, but Team Fortress 2 always gets at least one launch per year.

Dwarf Fortress, going on 15 years now!
Not every year, but I've played through Morrowind completely at least five times, And each time I'll spend a couple hundred hours easily in that world.
Red Dead Redemption 2. Still my favorite in game story of all time.
Classic Doom/Doom 2, as there's an endless number of wads and mods for it, I would say I play it even more than just once a year, its probably more like a few times a month
Mirror's Edge
Tetris, The Crew 2, Worms World Party / Reloaded and Tricky Towers - sometimes with my son (10). Yeah, looks like I'm old school hrhr
My all-time favorite is still Tetris - in all variants. For years I loved to play Kirby's Avalanche.
I also remember playing Duke Nukem 3D and Quake 3 Arena for a decade or so.
Duke Nukem 3D, Jazz Jackrabbit 2 and GTA 3, for me. They just feel cozy at this point...!
For me it's gotta be a driving game of some sort. Whether American/Euro Truck Simulator, or one of the Forza Horizons, or Dirt Rally. Whenever I'm itching for some escapism I'll play those games while catching up on podcasts. No Man's Sky as well
There is always a point in the year when I get life overwhelmed and turn to either Stardew Valley or a souls-like.
Single-player, open-world post-apocalyptic zombie survival roguelike. Always has new stuff being added to it and has for years. Come back after being away for a bit, and there's a ton of new stuff.
Dragon Warrior for the NES. Sometimes with game genie cheats, sometimes clean.
Also, Portal. It was a triumph.
I used to replay Kingdom Hearts 1 or 2 at least once a year. I've been pretty soured on the series since 3 so I haven't done it in years. Doesn't help I only play on PC now and the KH games are $60 each/Epic exclusives so I'm pretty disincentivized from buying them.
Always seems to be either spending 4 days adding mods to Skyrim, thirty minutes playing it then realising my computer sucks and doesn't like 200 mods and uninstalling it. Otherwise, Pokemon Gen1 and 2.
Skyrim, Morrowind, Runescape (Rs3) and minecraft seems to all pop up for at least one week a year since their release.

Sacred 2, always coming back to this one.
